WHAT DO I NEED TO REACTIVATE MY MC NUMBER?

 Please note that prior to requesting a refund, you must have your BOC-3 (Designation of Process Agent) form and your insurance before the refund can be processed. If they are not in place, your claim will be placed on hold until the BOC-3 and insurance requirements are met.

ALL REFUND REQUESTS MUST INCLUDE:

• CM number
• Carrier’s legal name
• Name of the person or name of the company on the credit card or check/money order
• Credit card billing address (if paying by credit card)
• A phone number where you can be reached
• Signature and date
• Email address if available
